Person-Centered Coaching is available when a barrier challenges the success of the person or the implementation of the Plan of Care. The person-centered coach is under the direction of a positive behavior support specialist or other licensed professional in the settings where the Plan of Care is implemented.
A PCC assists a participant and the participant’s person-centered team in:
- Implementing and monitoring the effectiveness of the participant’s person-centered plan of care;
- Modeling person-centered thinking;
- Training a participant, family, guardian, natural and unpaid supports, as well as other members of the person-centered team when barriers challenge the success of the participant in achieving his or her goals.
